What’s Included in My Medical Records?
Medical records contain essential information about your health history, diagnoses, conditions, and treatment plans. This includes medications you may have received while at one of our facilities.
Medical records are confidential, and your written authorization is required before a copy can be released.
Medical records may include, but are not limited to:
Demographics
Admission and discharge information
Diagnosis
Treatment plan
Safety/Crisis plan
Assessments
Psychiatric evaluations
Progress notes
Right of Access
You have the right to review and/or request a copy of your protected health information maintained by Healthy Life Choices, GA. You may also request that a copy be sent to a designated individual or healthcare provider of your choosing.
This right extends to personal representatives authorized to make healthcare decisions on behalf of a client.

Note: The Medical Records Department does not have access to billing records. Please contact the Billing Department for billing-related requests.
Third-Party Requests
If requesting records on behalf of a client, complete the Authorization to Release/Obtain Information form.

As these requests are made by representatives rather than the client, the Patient Right of Access rules do not apply.
Processing Time
All requests are processed within 30 days of receipt. If additional time is required (e.g., off-site records or incomplete authorization forms), we will notify you.
Costs
No charge for records sent directly to a healthcare provider for continuity of care.
A reasonable, cost-based fee may apply for personal requests.
State-regulated fees may apply for other use cases.
Healthcare Power of Attorney (POA) & Durable POA
Healthy Life Choices, GA recognizes valid POA and Healthcare Power of Attorney (HPOA) documentation. However:
The POA must explicitly allow the representative to make medical decisions on behalf of the patient.
The HPOA must include a physician’s letter confirming the patient lacks decision-making capacity.
POA/HPOA authority ends upon the patient’s passing.
